Primary Care Networks

A PCN is a group of GPs who have agreed to work together collaboratively with local service providers, social care and the voluntary sector, to respond to the needs of the patients they serve.  

Through PCN working we are able to offer patients additional services which include:

• Social Prescribers
• Physiotherapist triage
• Paramedic home visiting service
• Additional appointments provided by GPs, ANPs, Nurses & HCAs.
• Mental Health Practitioners
• Pharmacists
• Specialist Diabetes Dietitians.
• Dementia Admiral Nurses

Brinsworth and Whiston Medical Centre is part of the Raven Primary Care Network. This PCN is made up of 5 practices:

• Brinsworth and Whiston Medical Centres
• Gateway Primary Care
• Stag Medical Centre & Rose Court Surgery
• Thorpe Hesley Surgery
• Treeton Medical Centre

Due to the nature of collaborative working, services will be provided across all 5 sites. Therefore patients may be able to attend appointments at all 5 locations
